Community Care Packages

The aim of the Community Care Program is to maintain the independence of elders wishing to remain independent and to reside in the community for as long as possible.
To be eligible for Community Care Packages elders assessed as being appropriate for low care but elect to remain in their own home.

What services are provided by Community Care Packages?

Community Care Packages are available throughout each of Fronditha Care Inc sites. All Community Care Packages are Case Managed by English / Greek speaking Coordinators and trained Personal Care Staff.

Services include:

• Case Management;
• Assistance with personal hygiene, for example bathing and grooming; dressing and undressing;
• Light housework including laundry and ironing;
• Meal preparation;
• Transport and escort to the local shopping centre either as a shopping trip or as an outing.
• Transport and escort to medical appointments;
• Companionship and social activities;
• Respite care;

Other services may be available depending on the assessed needs of elders.
